Job Description
Basic Function:
Responsible for providing transportation using Paraquad vehicles as required for Paraquad participant transportation and other service delivery.  Prepares accurate documentation required by funding sources.  The individual in this role works approximately 29 or less hours per week.
 
Responsibilities:
 
Transportation
  • Transport participants, volunteers, and others as needed or as directed.
  • Make other transportation runs for various departments as approved by the Director of Facilities.
  • Ability to provide physical assistance to staff and participants when needed and appropriate, and when it does not interfere with the provision of transportation. 
  • Tidy up vehicles at the end of each shift.
 
Documentation
  • Document direct and indirect services with participants according to policies and procedures established by Paraquad and all regulating entities.
  • Keep daily records on van trips and complete monthly transportation report.
  • Make timely reports to supervisor on all non-routine events concerning any aspect of the Transportation Program, such as:  traffic violations, non-routine maintenance items, issues with a passenger, etc.
 
Miscellaneous
  • Complete special projects as assigned.
  • Protect participant’s rights to privacy and confidentiality in all forms of communication.
  • Attend and participate in all required trainings and meetings every fiscal year.
  • Maintain an understanding of and commitment to the independent living philosophy.

Education and Training
  • High school diploma or GED equivalent. 
  • Must have DOT physical examination as a condition of employment. 
  • Valid driver license and CDL or Class E license with no prior arrests or convictions. 
 
Experience
  • One-year experience in professional driving plus a clean driving record.
